PHARMACIST
In this role, the Pharmacist will be responsible for accurately dispensing medications prescribed by healthcare providers, educating patients on proper medication use, and providing expert advice to physicians regarding medication selection, dosage, interactions, and potential side effects. Depending on circumstances, the Pharmacist may also exercise delegated decision-making authority within the scope of their licensure.
QUALIFICATIONS
RESPONSIBILITIES
Medication Management: Review and interpret medication orders, ensuring accuracy and appropriateness of prescribed therapies. Monitor patient medication regimens for safety, efficacy, and compliance. Verify physician medication orders with accuracy and timeliness. Monitor and evaluate the six critical processes of medication management; determine risk points and is an active participant with physicians and clinical staff in continuous quality and performance improvement Patient Counseling: Provide medication counseling to patients, educating them about their medications, potential side effects, interactions, and proper usage to promote adherence to prescribed therapies. Collaboration: Work collaboratively with healthcare providers, including physicians, nurses, and other members of the care team, to develop and implement comprehensive medication management plans. Clinical Services: Participate in clinical programs and initiatives aimed at improving patient outcomes, including medication therapy management, chronic disease management, and immunization programs. Quality Assurance: Ensure compliance with all federal, state, and local regulations governing pharmacy practice. Participate in quality improvement initiatives to enhance pharmacy services. Inventory Management: Manage and oversee the procurement, storage, and dispensing of medications, ensuring that inventory is maintained in accordance with safety standards and regulatory requirements. Education and Training: Provide education and training to staff and patients on medication use and safety. Participate in the training of pharmacy interns and students, if applicable. Verify and approve the work of Pharmacy Technician(s) Record Keeping: Maintain accurate and up-to-date records of medication dispensing, patient profiles, and pharmacy operations in accordance with legal and regulatory requirements.

QUALIFICATIONS
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Active pharmacist license in California.
- Previous experience in a clinical pharmacy setting, preferably in an FQHC or community health environment.
- Strong knowledge of pharmacotherapy, drug interactions, and therapeutic guidelines.
- Excellent communication skills, with the ability to work effectively with a diverse patient population.
- Commitment to providing culturally competent care and addressing health disparities.
- Ability to work collaboratively in a team-oriented environment.
