Job Title: Licensed Vocational Nurse / Licensed Practical Nurse

Job Specialty: Long Term Care

Shift: Nights, 8 hours

Guaranteed Hours: 40 hours per week

Experience: 3+ years of current experience within the last 6 months

License: Current New Hampshire license

Certifications: BLS

Must-Have:
- Proof of COVID vaccination series and booster
- Willingness to work Christmas through New Year Day block, if needed/scheduled
- Ability to float
- Proof of transportation due to limited public transport availability

Job Description:

- Observe each resident continuously and be alert to changes in their mental and physical well-being. Notify the Supervisor of pertinent observations and discuss the need to notify a physician. Receive Resident Status Report from the off-going shift and provide a detailed report to the on-coming shift about each resident's status and events.
- Accurately receive, transcribe, and carry out physician orders. Administer medications and treatments as ordered and properly document them using a computerized system.
- Supervise LNAs in accordance with state licensing requirements. Assign and measure nursing staff resident care consistent with abilities, job description, and scope of practice. Assist direct-care staff with nursing care as necessary and as time allows.
- Accompany physician on resident rounds on assigned unit(s) to address residents' medical needs and facilitate communication during 60-day reviews.
- Administer oxygen provision, tube feedings, and perform wound care according to facility guidelines and procedures.
- Monitor appointments for the day, ensuring checks on therapy appointments, activity board, and remind staff of ambulation lists and other tasks.
- Enforce infection control practices and ensure the unit is clean and hazard-free. Report any problems through the chain of command.
- Complete 24-hour reports, check new orders, and maintain consistent communication with the Nurse Manager regarding resident issues or concerns with staff.
- Communicate with residents' families to resolve concerns and notify them of new orders or necessary consents.
- Assume responsibility for the management of assigned unit(s) under the direction of the Nursing Supervisor or Director of Nursing Services.
- Follow regulations regarding controlled medications and assist with the orientation of new nursing employees.
- Encourage and assign staff to attend in-service education meetings and assist with staff education and training as needed. Evaluate nursing staff members and recommend disciplinary action as necessary.
