Job Description:
- Function independently to provide high-quality patient care through a collaborative and patient-centric approach.
- Provide care in various settings and modalities, including direct interaction with patients in hospitals or clinics, as well as via telephone, video, or secure email when appropriate.
- Collect patient history, conduct physical exams, review prior records, and analyze ancillary data using sound judgment and critical problem-solving skills.
- Develop and monitor appropriate treatment plans, communicating effectively with patients, their families, and the care team.
- Provide efficient, compassionate care utilizing a multidisciplinary approach through communication, continuity, coordination, and evaluation.
- Offer relevant education and counseling to patients and families to support care plans, treatment, and discharge.
- Collaborate with other healthcare team members and medical staff to ensure safe patient care and communicate pertinent findings to the physician and team.
- Assist collaborating physicians with patient management in the hospital, in line with care site policies, regulations, and approved scope of practice.
- Ensure compliance with regulatory standards and practice metrics, and document according to regulatory and care site guidelines.
- Promote the mission, vision, and values of the healthcare organization.
- Perform other assigned duties, with the potential to float to other departments based on business needs. May require being on-call during a scheduled shift.
- Ability to work nights and/or weekends as required.
