Shift: 08:30 AM - 04:30 PM

Guaranteed Hours: 35 hours per week

Experience: Minimum 1 year of professional SNF experience

License: Must hold a valid license

Certifications: Relevant certifications required

Must-Have:
- Strong professional SNF experience
- Ability to achieve 90% productivity after 2 weeks

Job Description:
- Provide skilled occupational therapy services to patients in a SNF setting.
- Develop, implement, and evaluate individualized patient care plans.
- Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
- Document patient progress and treatment outcomes in a timely manner.
- Maintain compliance with professional and regulatory standards.
- Educate patients and families about continued care and preventive measures.
- Perform assessments and adjust treatment approaches as needed.
