Job Description
1. Supervises and/or coordinates the operation of a Pharmacy unit or major function.
2. Prepares departmental reports, including the performance of statistical analyses.
3. Develops and implements new processes to improve the operation of the Pharmacy and the quality of care provided.
4. Orients new staff members.
5. Monitors and evaluates staff performance and follows appropriate disciplinary proceedings, as necessary.
6. Prepares unit work schedule.
7. Ensures staff compliance with regulatory agency and health care setting rules and regulations.
8. Responsible for the maintenance of adequate quantities of medication and other supplies in the Pharmacy unit.
9. Evaluates new drugs released by the FDA in terms of efficacy, adverse effect profile, and cost;
makes recommendations on additions and deletions to the health care setting's drug formulary.
10. Manages the medication therapy of a group or unit of patients as part of an interdisciplinary team, and participates in rounds.
11. Coordinates the assessment of medication use.
12. Serves as a professional and technical resource for medical professionals in specialty area(s).
General tasks and responsibilities will include:
Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols
Provides team based care on the nursing unit - verification of medication orders, participation in White Board Rounds; medication reconciliation, patient counseling and answering drug information questions.
Issues controlled substances to patient care areas and maintains records as required by law
Ensures safe, appropriate, cost-effective therapies for patients according to established policies, procedures and protocols.
Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug, dose and therapeutic duplications.
Detects and reports adverse drug reactions and medication errors accurately and in a timely manner
Sustains the formulary by minimizing non-formulary procurements utilizing therapeutic substitutions and promoting rational medication therapy selections
Provides accurate, adequate and timely drug information to families and staff.
Provides drug information to families and staff
Documents all clinical activities and interventions accurately and completely
Participates in the quality, performance improvement and medication use review activities of the department
(collects data, conducts inspections, and maintains logs, records and .other documentation as required.
Compounds sterile and non-sterile products
Participates in the training of new pharmacy staff members
Supports the HSP LVL III inpatient satellite manager with reports
Provides coverage as requested by the Director or Assistant Directors
Minimum Qualifications
1. Possession of a valid NY State license to practice pharmacy.
2. Two years of post licensure experience; or
3. Completion of a post graduate specialty residency or fellowship program; or
4. Current Pharmacy Board Certification.
