Job Title: Case Manager RN
Job Specialty: Case Management
Job Duration: 13 weeks
Shift: Friday-Monday, 10-hour shifts
Guaranteed Hours: 40 hours weekly
Experience: 1-2 years minimum, 5 years preferred
License: Compact state RN
Certifications: AHA BLS, CCM/ACM preferred
Must-Have: Utilization management, utilization review (InterQual), discharge planning, insurance, acute hospital case management experience, recent traveler dyad case management experience
Job Description:
- Participate in weekly orientation sessions starting on Mondays.
- Work in a Joint Commission Accredited facility with a reputation for safety and quality care.
- Focus on utilization management, discharge planning, and adherence to CMS guidelines.
- Employ skills in InterQual and MorCare (preferred) for effective case management.
- Manage a diverse patient population including geriatric, adult, adolescent, women, and infants.
- Handle common diagnoses like CHF, Sepsis, COPD, Pneumonia, UTI, CVA, and Acute Renal Failure.
- Work collaboratively as a team player, demonstrating empathy and resourcefulness.
- Follow the dress code policies, avoiding jeans or shorts.
- Advanced proficiency with laptops and dual monitors, and familiarity with Paragon EHR is required.
This role is intended to replace permanent staff and will not require floating or on-call duties. Responsibilities focus heavily on discharge planning and patient transition management. Preferred candidates should be experienced in utilizing InterQual and maintaining compliance with Conditions of Participation.
Note : Bachelor's degree in Nursing is preferred, although an Associate's degree is required.
