Nurse Practitioner - Medicare Annual Wellness

Position Overview:

We are seeking an experienced Nurse Practitioner to join our healthcare team, primarily supporting Medicare Annual Wellness patient visits. The Nurse Practitioner will deliver comprehensive, cost-effective healthcare services while maintaining the highest standards of advanced nursing practice.

Job Responsibilities:

• Conduct Medicare Annual Wellness visits, performing thorough patient assessments and developing personalized prevention plans
• Provide comprehensive family healthcare services, including patient assessment, diagnosis, and treatment
• Collaborate with the healthcare team to ensure optimal patient outcomes
• Maintain accurate and detailed patient records

Required Qualifications:

Education:

Master's Degree in Nursing from an ANA accredited program

Graduate of an accredited Nurse Practitioner program

Experience:

Minimum of 2 years of clinical experience as a Nurse Practitioner

Licensure and Certifications:

Current national certification as a Family Nurse Practitioner

Active Massachusetts state nursing license

Current DEA and Massachusetts Controlled Substance license for prescriptive authority

Current Basic Life Support (BLS) certification

Reporting directly to the Medical Director of Primary Care Services
