Job Description
Job Title: Physical Therapist
Job Specialty: PT Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year Adult Home Health Experience required; First-timers not accepted
License: Must have license in hand unless it takes a few days to obtain. Pending licenses are not accepted.
Certifications: CPR and BLS required
Must-Have:
- Reliable transportation
- Current driver's license
- Agency-required liability insurance
- Proficient in OASIS and HomeCare HomeBase EMR, used within the last 6 months, or completed a retraining course
- Ability to travel up to 75 miles from home clinic
- Willingness to accept every patient on schedule
- Documentation completed within 24 hours of appointment
Job Description:
- Perform home visits for therapy and rehabilitation services
- Engage in periodic office meetings
- Manage an average of 5-6 patients per day, equating to roughly 6-8 points per day
- Cover regions including The Hamptons, Williamsburg, Hatfield, Hadly, Amherst, Pelham, and surrounding areas
- Workload includes Start of Care and Follow-Ups
- Participate in weekend rotation every 5th weekend
- Engage in travel approximately 150 miles per week
- Complete weekly productivity expectations of 30 points
- Responsible for travel documentation from patient home to patient home, with mileage reimbursed at the IRS standard rate
- Not responsible for commute to and from the first and last patient
- Opportunity to float to nearby clinics if more hours are needed to meet financial needs.
