General Summary of Position

Job Summary:

MedStar Health is looking for a Regional Program Manager Community Health to join our team at MedStar Union Memorial Hospital. We are looking for someone with strong analytical and critical thinking skills, strong community engagement and facilitation skills, and advanced project management skills.

The Regional Program Manager Community Health will manage the day-to-day coordination, administration, community engagement, and data functions for the Population Health Initiatives, under the direction of the Regional Director of Community Health.

Primary Duties:

  • Leads in selecting, training, orienting, and assigning department staff. Develops standards of performance, evaluates performance, and conducts performance management planning. Initiates or makes recommendations for personnel actions. Oversees workforce staff including Community Health Advocates, Safe Streets Hospital Responders, and other community health worker initiatives as applicable. Maintains ongoing communication with office staff to review programs, provide feedback, discuss new developments, and exchange information.


  • Acts as primary agent for the coordination and implementation of the Population Health initiative. Assists Case Management and other hospital departments to implement outreach efforts that engage patients to adhere to care plans and/or remove barriers to care. Assists in the synthesis of data analysis into clear, relevant, and visually appealing materials that can be used by the Working and Strategy Groups to make strategic decisions. Leads grant application processes to allow the hospital to address social determinants of health that tie to community health needs assessments; manages and facilitates teams to complete and lead multidisciplinary teams.


  • Coordinates advocacy supporting activities of partners. Coordinates outreach, research, and other activities with initiative partners to minimize redundancies and to align strategies and actions, including conducting individual outreach and education as needed. Coordinates with other related projects and coalitions and integrates their work into Steering Committee and Working Group processes. Creates and manages summary reports of Population Health Initiative progress for Steering Committee and external audiences.


  • Develops communications materials for Population Health Initiative as the program evolves, potentially including summary documents, brochures, FAQs (Frequently Asked Questions), and other items; web and social media strategy; press strategy, including drafting press releases. Facilitates ongoing communication amongst partners by hosting regular conference calls, in-person meetings, or coordinating regular email updates to ensure alignment of activity. Launches social needs assessment screening, embedding in clinical workflows across hospitals.


  • Oversees high quality data management systems and uses historical information regarding programs to lead and form more robust outcome measurements for evaluation of programs. Participates in analysis of data to glean useful insights for the Working Group, Strategy Groups, and other local and statewide audiences. Plans, manages, and attends community activities. Presents data intelligently and creatively in a way that can be easily and quickly grasped by external stakeholders wishing to learn more about the initiative. Provides support to partner organizations in aligning their work to the Common Agenda.


Qualifications:

  • Bachelor's degree in public health, Nursing, Social Work, or related field.


  • 3-4 years' experience including 1+ years of proven case management, project management and population health coordination experience.


  • Familiarity with the local area and/or population health workforce integration.


  • Community outreach experience preferred.


  • Experience with data collection and reporting preferred.

This position has a hiring range of $70,428 - $131,310
