License: RN

Job Description:
- Provide specialized nursing care in a home health setting.
- Coverage area includes the Valley service area.
- Travel requirement ranges from 50-100 miles per day, depending on patient needs.
- Utilize common equipment and charting systems, including EPIC and Remote Client.
- Implement patient care plans and ensure continuity of care.
- Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records.
- Educate patients and their families on health maintenance and disease prevention.
- Maintain a collaborative relationship with interdisciplinary teams.
- Ensure compliance with all health regulations and protocols.
