Job Description
Job Title: Registered Nurse

Job Specialty: Home Health

Shift: Day - 5x8 - 09:00 AM - 05:00 PM

Guaranteed Hours: 40 Hours per week

Experience: Minimum of 2 years in a home health setting within the last 10 years

License: Current RN Licensure in New York State

Certifications: Basic Life Support (BLS) required before the start date

Must-Have:
- Two years of experience in a home setting, which may include a Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC)
- Conducting assessments similar to UAS such as OASIS
- Providing primary care to homebound individuals
- Experience in care management for individuals receiving home and community-based services, including hospice and assisted living
- Home care experience as a paraprofessional or LPN, caring for a minimum of 2 patients per week

Job Description:
- Perform initial in-home pre-evaluation of patients, including health status evaluation, assessment of strengths, care needs, and preferences, and guide the development of individualized long-term care service plans.
- Review consumer medical documentation and health referral forms as relevant to the case.
- Enter evaluation data into an electronic evaluation form and transmit as required.
- Document any concerns, conflicting information, or other issues that arise during the evaluation process.
- Emphasize continuity of care to reduce or eliminate fragmentation, duplication, and gaps in treatment plans.
- Discuss healthcare options, required support, service vendor options, and waiver options with consumers.
- Initiate communication between the call center and eligible consumers for enrollment into a managed care health plan.
- Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery.
- Collect quality review data and any required documentation to support outcome measurements and record case notes into CRM.
- Identify opportunities for health promotion and illness prevention.
- Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population.
- Meet all standards outlined in the corresponding annual performance criteria and bonus template for the position.
- Perform other duties as assigned by the Regional Nurse Manager or project management.
