Job Description

Purpose of Position:

Under the guidance and direction of the Assistant Director of Nursing and Director of Nursing, the Wound Care RN contributes to developing, implementing, and evaluating the nursing regimen for assigned patients on his/her unit. A thorough knowledge of nursing theory, principles, and standards of quality nursing care guides the wound care RN.

The Wound Care RN is responsible for managing patients requiring treatment and is held accountable for using the nursing process. She/He is responsible for implementing the Nursing Department Philosophy and Objective of Nursing care into managing nursing care in patients' units.

RESPONSIBILITIES:
  • Implements the wound and ostomy care protocols, documentation process, and wound and ostomy care formulary following Wound Ostomy Continence Nursing (WON) and National Pressure Ulcer Advisory Panel (NPUAP).
  • Renders individualized comprehensive nursing care based on the nursing process to meet the patient's physical, psychological, spiritual, social, and educational needs.
  • Comprehensive assessment of patients at risk for developing pressure ulcers.
  • Document wound and/or ostomy assessment, including measurement, staging, treatment, and outcomes.
  • Prepares, administers, and documents administration of wound treatment.
  • Coordinates patient care by collaborating with a multidisciplinary team, provides wound consultation to nursing staff and physicians, and acts as a clinical resource person to the staff.
  • Assess each patient's needs and establish an appropriate plan of care based on Nursing
  • Assessment and diagnosis.
  • Supervises and assists the Staff in patient assessment, documentation, implementation, and evaluation of nursing care.
  • Provides direct care education and consultation to patients and families.
  • Utilizes the nursing process to plan, implement, direct and evaluate patient care to achieve optimal patient outcome.
  • Makes rounds with the team and participates with the team in developing priorities for patient care.
  • Collaborates interdisciplinary with all health care workers in the delivery of patient care.
  • Notifies physicians of patients requiring wound debridement.
  • Responsible for reporting to the ADN &/or DON any situation or problem on the service that impedes nurses in performing their duties using an appropriate handoff tool.


Minimum Qualifications
1. Valid license and current registration to practice as a Registered Professional Nurse issued by the New York State Education Department (NYSED); and
2. Current certification in one of the following:
Certified Wound Ostomy Continence Nurse (CWOCN) issued by the Wound, Ostomy and Continence Nursing Certification Board (WOCNCB); or
Certified Wound Ostomy Nurse (CWON) issued by the WOCNCB; or
Certified Wound Care Nurse (CWCN) issued by the (WOCNCB); or
Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM); or
Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO).
3. Holds or obtains a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
