Key Responsibilities:
- Conduct initial client assessments to determine care needs
- Develop and implement individualized client care plans
- Case management, including building longlasting relationships with assigned clients
- Perform skilled nursing duties such as wound care and medication management
- Perform re-assessments and care plan updates to maintain the client's optimal health
- Orient caregivers at the start of a case as applicable
- Provide in-person and on-call support to clients as needed
- Maintain client records and documentation
- Educate clients and families about healthcare management options
- Maintain accurate documentation of nursing care provided
- Conduct caregiver competency assessments as needed
- Supervise and visit field staff as necessary
Requirements:
- Valid New Jersey Registered Nurse (RN) license
- CPR/Basic Life Support certification
- Proof of eligibility to work in the U.S
- Proficient computer literacy
- Valid driver's license and reliable car
- Ability to work independently and as part of a team
- Strong clinical and critical thinking skills
- Exceptional communication and interpersonal skills
- Ability to lift up to 50 lbs
- One year of nursing experience preferred
- Home care experience preferred
