Job Description:

- Provide home health care services within the Valley service area.
- Travel ranges from 50 to 100 miles per day, depending on patient needs.
- Utilize EPIC and Remote Client systems for patient charting.
- Ensure all patients receive high-quality nursing care in their homes.
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Monitor patient progress and adjust care plans as necessary.
- Educate patients and their families on treatment plans and health management strategies.
- Maintain accurate and timely documentation of patient care.

This position requires a compassionate and driven Registered Nurse dedicated to providing exceptional care in a home health setting.
