Job Title: Social Worker LMSW Case Management

Job Specialty: Acute Social Work

Job Duration: 15 months

Shift: Full Time Days, Weekends/Holidays as needed, Monday to Friday, 8:30 a.m. - 4:30 p.m.

Experience: Minimum two years of related experience in healthcare, including counseling and discharge planning. Preferred experience in acute hospital settings, nursing home knowledge, and conducting crisis intervention evaluations.

License: Licensed Master Social Work (LMSW) or Licensed Clinical Social Worker (LCSW) in New York State or eligible for New York State license.

Certifications: Minimum Screen certification or experience or intent to pursue within six months.

Must-Have:
- Ability to handle confidential information with discretion and professionalism.
- Capacity to meet deadlines, manage multiple priorities, and model teamwork effectively.
- Excellent interpersonal, communication, and organizational skills.
- Computer literacy is essential.

Job Description:
- Identify patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge.
- Provide high-quality, evidence-based care to patients while promoting cost-effective utilization of hospital resources.
- Collaborate with the interdisciplinary team to develop and implement integrated discharge plans for positive patient and family outcomes.
- Make appropriate referrals for community services and provide emotional support, therapeutic interventions, crisis intervention, bereavement services, and patient/family education.
- Assess and intervene in situations involving mandated reporting.
- Supervise graduate social work student interns.
- Identify and address barriers to accessing primary and preventive services for patients presenting with non-emergent needs.
- Educate patients on how and where to access healthcare services, including the roles of primary care providers, urgent care, and emergency departments, and refer to health home care managers as needed.
- Educate patients on transportation resources in the community.
