Job Description
SUMMARY OF DUTIES AND RESPONSIBILITIES
Under the direction of the Deputy Executive Director for Ambulatory Care, oversee all aspects of the Primary Care and Medical Practices as well as the HIV/AIDS Grants, Programs and services. Work collaboratively with the Ambulatory Care leadership as well as the leadership at NYC H+H Kings County. Ensure that the Patient-Centered Medical Home (PCMH) Teams are functioning well, meet their monthly metrics and successful submit the annual application for certification. Ensure that the Community Health Workers and the ACO is incorporated into Primary Care and the PCMH Teams. Ensure that the fiscal viability of the Primary Care Practices, HIV Services and Ryan White and Grant Funded Programs are maintained and meet financial viability. Participate in all of the Breakthrough Initiatives and ensure that the staff are fully engaged. Plans, directs, supervises and reviews work of subordinate administrative personnel relating to assigned tasks and projects. Provides administrative supervision, coaching and support to managers as well. Responsible for the identification, corrective plan development and execution of all patient flow issues, ensuring that the administrators responsible are held accountable to develop and complete the plans in a timely manner. Together with the Deputy Executive Director of Ambulatory Care, works collaboratively with Ambulatory Care's Nursing Leadership and Directors of Service to develop and implement solutions to increase patient and staff satisfaction. Collaborates with physicians, nursing and other health care professionals in establishing, implementing and maintaining patient care. Quality, and service standards to meet patients' and internal clients' expectations with regards to provider access and provider choice. Develops and evaluates processes to improve patient cycle time and patient care results across the continuum of care. Collects and analyzes data for administrative and fiscal reports, budget preparations, operational data and related support services. Monitors fiscal performance and identifies and implements strategies to reduce costs, increase revenue and improve quality of care. Determines the appropriate non-clinical staff-mix for multiple Practices/Programs and manages the Human Resource functions; develops processes to screen, interview, recommend for hire, train and maintain the competency of all staff. Responsible for the successful implementation and spread of the Daily Management System (DMS)throughout all of the Practices. Directs the development, implementation, and monitoring of access related policies and procedures which support the Organization's goals and business objectives. Collaborates with Physician Team Leaders and Department Chiefs in planning and developing the level of patient service/programs, provider schedules, and the day-to-day operations of the Practices.
Minimum Qualifications
1. A Masters Degree from an accredited college or university and seven years of fulltime
paid experience of program development in a hospital or health care facility in
meeting community health needs, health care planning, financial planning, operations and analysis and implementation, in an administrative, consultative, managerial or
executive capacity; or,
2. A Baccalaureate Degree from an accredited college or university in Business
Administration, Social Sciences, Management, Health Care Administration or related
discipline; and,
3. Eight years of full-time paid experience at the level indicated in (1) above; and,
4. Demonstrated skills in written and medical communications; or,
5. A satisfactory equivalent combination of education, training and experience.
