Must-Have: Candidate must be reviewed and pass clinical assessments, have a flexible schedule including working major holidays as needed.
Job Description:
- Assess, evaluate, and drive the discharge plan.
- Coordinate services to match the patient's appropriate level of care.
- Ensure that the treatment plan is initiated, evaluated, and reassessed daily.
- Monitor progress or lack thereof on a daily basis.
- Track the length of stay and identify barriers to discharge.
- Follow Medicare regulatory guidelines regarding patient choice, IMM notices, and MOONS.
- Work schedule: Monday through Friday, with no weekend or on-call requirements.
- Floating within facility/scope of practice as needed.
- Compliance is required for an on-time start, with a possible two-week delay for non-compliance.
- The candidate must be a traveler away from certain facilities for one full year to reapply.
- Supervisory references are highly preferred.
- Candidates must be available to submit core documents and complete/schedule labs within 48 hours of contract signing to ensure an on-time start.
