Your Role as an RN Case Manager
As a Registered Nurse Case Manager at Ancora, you will:
Deliver exceptional home health & hospice care, ensuring physical, social, and emotional well-being
Perform comprehensive patient assessments and create individualized care plans
Collaborate with patients, families, and the interdisciplinary care team to develop and adjust treatment strategies
Coordinate patient care delivery, manage resources efficiently, and maintain accurate patient records
Serve as an advocate and resource for patients and their families
Provide end-of-life support with dignity, respect, and compassion
Proactively initiate referrals and coordinate with other healthcare professionals
What We're Looking For
Current Registered Nurse (RN) License in the state of Alaska
Home Health or Hospice experience (preferred)
Ability to perform full range of movement for patient transfers
Strong verbal & written communication and time-management skills
Valid driver's license, clean driving record, and reliable transportation
Proof of current auto insurance & vehicle registration
Fingerprint clearance card (we can assist with obtaining one)
CPR certification & negative TB test (we can help with these as well)
