About The Opportunity
Working as an integral part of the Yustway Health and Wellness Department reporting to the Manager of the Home and Community Care (HCC) division, the Team Lead RN, Home and Community Care supports the Manager to ensure smooth daily operations of the HCC program and acts as the first point of contact and direct supervisor of the HCC Nurse Assessors, Senior HCA, and the Client Care Coordinator. The individual ensures the provision of quality home care services, proper implementation of Client Care Plans, exceptional client service standards, and client access to other health care professionals. This senior level position requires a professional, skilled, and gentle leader who has exceptional knowledge and skills in home care client services, management and administration, people leadership, scheduling, policy adherence, and records management. The incumbent must have a strong understanding of the British Columbia's regulations associated with Home and Community Care including delegated and assigned tasks, client and staff safety and confidentiality. The individual promotes a culturally safe, supportive, empowering, and therapeutic environment for Nation Members, clients and families, colleagues, and partners. They practice from a place of respect for, and a willingness to learn about, the Squamish worldview that recognizes and honours the values, beliefs and customs of individuals, families, and the community, as well as upholding cultural safety standards and best practices current to the field of home care.
What You Bring
Salary: $ 100,477 - $ 114,179 annually, plus a comprehensive benefits program

What You Bring
- Combination of relevant experience, education, and training.
- Knowledge of concepts, tools, practices, and methods related to health services accreditation and quality improvement, system design, and change management an asset.
- Working knowledge of community-based health policy, programming, and service leadership.
- Team-oriented with a positive, approachable attitude.
