Hospice & Palliative Nurse Practitioner

Trellis Supportive Care is currently seeking a full time hospice and palliative care Nurse Practitionerto join our team of medical professionals to provide compassionate medical care to all patients, including pain and symptom management, admissions and eligibility reviews, face to face visits, and chart reviews. The Nurse Practitioner will provide palliative and/or hospice care visits at patient's home, in long term care facilities, in our inpatient facility, or wherever the patient calls home. In addition, the Nurse Practitioner will work closely with the clinical teams to ensure quality care and act as a resource to the clinical team to discuss patient care issues.

We have made it our mission to provide compassionate care for individuals living with a life-limiting illness and their families, through quality medical, emotional, spiritual, and social support wherever care is needed. Our staff has grown from a dozen individuals to over 350 specially trained professionals who work as a team to help patients live fully and comfortably, with hope - even during life's most challenging time.

Qualifications:
  • Current license as Nurse Practitioner with the North Carolina Board of Medicine & North Carolina Board of Nursing. National certification required after January 1, 2000.
  • Hospice or homecare experience preferred.
  • Excellent nursing assessment, problem solving skills, and interpersonal communication skills.
  • Demonstrates excellent critical thinking skills with team oriented approach and a dedication to providing compassionate end of life care for the communities we serve.
  • Sensitive to the needs of terminally ill patients and families and one's own feelings about death and dying. Ability to cope with emotional stress and be tolerant of individual lifestyles.
  • Exceptional customer service required.
