The referenced pay range represents the minimum and maximum compensation for this job. Responsibilities:
GENERAL PURPOSE:
Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.
40 hours. No weekends. No Holidays.
The Epilepsy RN Navigator is an experienced registered nurse responsible for coordinating clinical care for patients with epilepsy. Collaborating closely with both adult and pediatric neurology multidisciplinary teams, the RN Navigator helps establish and manage comprehensive care plans, including preparing qualified patients for surgical interventions aimed at reducing seizure activity. This role also includes participating in continuous quality improvement initiatives to enhance patient outcomes. The RN Navigator systematically collects and assesses patient health information to develop, implement, and evaluate personalized care plans for both patients and their families. The ideal candidate will be independent, eager to learn, and committed to providing high-quality care. A current New York State RN license is required.
