Job Description
Position Overview:
The Renal Physician Assistant (PA) provides comprehensive care to patients with renal-related conditions, including chronic kidney disease (CKD), end-stage renal disease (ESRD), dialysis patients, and those requiring renal transplant management. The PA will work closely with the nephrology team, including nephrologists, nurses, social workers, and other healthcare professionals to deliver high-quality care in inpatient and outpatient settings at Lincoln Medical Center.
Key Responsibilities:
1. Clinical Responsibilities:
o Conduct comprehensive patient assessments, including history-taking, physical exams, and interpretation of diagnostic tests (e.g., kidney function tests, electrolytes, imaging studies).
o Develop and implement treatment plans for patients with kidney-related diseases, including CKD, ESRD, and complications related to diabetes and hypertension.
o Provide ongoing management of dialysis patients, including both hemodialysis and peritoneal dialysis modalities.
o Monitor patients undergoing renal transplantation, including post-transplant care and immunosuppressive management.
o Prescribe medications and monitor their effects on renal function, adjusting treatment protocols as needed.
o Provide pre- and post-operative care for patients undergoing renal-related procedures (e.g., dialysis access placement, kidney biopsies).
o Educate patients and their families about renal disease progression, treatment options, and self-management strategies.
2. Patient Care Coordination:
o Collaborate with multidisciplinary teams to ensure coordinated care for patients in the nephrology clinic, inpatient units, and dialysis units.
o Provide timely updates to attending nephrologists and other team members regarding patient progress, complications, and changes in clinical status.
o Coordinate referrals to specialists, social services, and other support services as needed.
o Serve as a resource for healthcare providers regarding best practices in the management of renal diseases.
3. Documentation & Reporting:
o Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
o Document patient encounters, treatment plans, and progress notes in compliance with hospital and regulatory standards.
o Participate in quality improvement initiatives related to patient care outcomes, documentation standards, and patient safety protocols.
4. Patient Education:
o Provide patient education on renal disease prevention, dietary restrictions, medication adherence, and lifestyle modifications.
o Offer counseling on dialysis options, kidney transplantation, and potential complications related to chronic kidney disease.
o Conduct educational sessions for patients with newly diagnosed renal conditions or those transitioning to dialysis.
5. Professional Development:
o Maintain current knowledge of nephrology trends, guidelines, and best practices through continuing medical education (CME), conferences, and workshops.
o Engage in regular self-assessment and professional growth opportunities, participating in peer reviews and feedback sessions.
6. On-Call & Weekend Coverage:
o Provide after-hours and weekend coverage as part of a rotating on-call schedule for the nephrology department, addressing acute renal-related issues in the hospital setting.
o Respond to urgent consultation requests for inpatient renal patients, including those in the ICU and emergency department.
Minimum Qualifications
1. Current New York State registration as a Physician Assistant; and,
2. Current certification issued by the National Commission on Certification of Physician Assistants (NCCPA); and,
3. Two years of full time, satisfactory clinical experiences as a Physician Assistant.
