Must-Have:
- Two years of experience in a home setting within the last ten years, including work with Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care.
- Experience conducting comprehensive assessments of patients.
- Ability to review health status questions and make evaluations based on criteria for enrollment.
- Proficient with workplace technology.
- Knowledge of general professional nursing theory, principles, and practices.
- Understanding of state and agency laws governing nursing practices.
- Excellent organizational, interpersonal, written, and verbal communication skills.
- Ability to communicate effectively with consumers from multiple generations.
- Sensitivity to the concerns of others.
- Capability to perform in a fast-paced, deadline-oriented environment and execute complex tasks.
- Strong ability to work independently.

Job Description:
- Perform initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences to guide individualized long-term care service plans.
- Review consumer medical documentation and health referral forms as relevant.
- Enter evaluation data into electronic forms and transmit as required.
- Document any concerns, conflicting information, or issues surfaced during evaluations.
- Emphasize continuity of care to reduce or eliminate fragmentation, duplication, and gaps in treatment plans.
- Discuss healthcare options with consumers, including support needs, service vendor options, and waiver options.
- Facilitate communication between the call center and eligible consumers for managed care health plan enrollment.
- Enhance collaborative relationships with interdisciplinary care team members to improve care coordination and service delivery.
- Collect quality review data and required documentation to support outcome measurements and record case notes.
- Identify opportunities for health promotion and illness prevention.
- Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population.
- Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template.
- Perform other duties as assigned by the Regional Nurse Manager or project management.
