Job Title: LPN - Full Time Day/Evening Rotation

Shift: Full Time Day/Evening rotation (5x8s), Every Other Weekend, Holiday 5x8

Experience: 1 year of experience as an LPN in a Long-Term Care Facility

License: LPN License

Certifications: Basic Life Support (BLS) certification through American Heart Association (AHA) or Resuscitation Quality Improvement (RQI)

Must-Have:
- LPN License
- BLS certification
- Experience in a long-term care setting

Job Description:

- Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served.
- Demonstrate knowledge of the principles of growth and development of the life span.
- Possess the ability to assess data reflective of the patient's status and interpret the appropriate information.
- Provide the care needed by adult and geriatric patient groups.
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
- Maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
- Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
- Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnoses and interventions to achieve desired resident outcomes.
- Recommend revisions to the plan of care after evaluation of resident outcomes, and indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
- Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice, assuring customer satisfaction.
- Maintain current knowledge and competency in Nursing Practice and participate in the mentoring/orientation of staff.
- Consider factors related to safety, cost, and impact in planning the delivery of care.
- Provide leadership in the professional practice setting under the direction of the RN.
