Job Description
Job Title: MDS Coordinator
Job Specialty: Nursing
Job Duration: 13 weeks
Shift: 8:00am - 5:00pm
Guaranteed Hours: 40 hours per week
Experience: Long-term care MDS coordinator experience
License: RN nursing licensure for state of practice
Certifications: Certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 months of hire
Must-Have:
- Graduate of an accredited school of nursing
- Knowledge of the RAI/MDS process
Job Description:
- Provide care to assigned residents in a caring, safe, and efficient manner according to location standards, procedures, and individualized resident care plans.
- Complete assigned portions of the MDS for Nursing and sign the MDS forms upon completion.
- Maintain residents' self-esteem, privacy, and confidentiality of personal information.
- Assist in maintaining safe, pleasant, orderly resident living areas.
- Complete the nursing portion of the MDS and review the entire process for completion and validity.
- Query other members for accuracy of MDS and provide a final quality check prior to MDS transmission.
- Observe and report residents' conditions and communicate with physicians using center/campus systems.
- Direct and assist with co-worker, resident, family/significant other education.
- Provide support and recognition to co-workers related to completion of the RAI/MDS process.
- Use the EMR accurately.
- Comply with all related laws, regulations, company policies, and procedures.
- Perform other duties as required, with responsibilities and activities potentially changing with or without notice.
Physical/Mental Requirements:
- Ability to communicate and comprehend, effectively communicate in English, both orally and in writing.
- Must be able to lift up to 25 lbs.
- Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
Work Environment:
- The noise level in the work environment is usually moderate.
- Must be able to work the floor as an RN if staffing gaps arise.
