Job Description
Job Title: Registered Nurse (RN)
Job Specialty: Home Health & Hospice
Job Duration: 13 weeks
Shift: 9H Days + Call - Monday to Friday 8 AM to 5 PM + On-Call rotation evenings/weekends, holiday coverage required
Guaranteed Hours: 40 hours per week
Experience: Minimum of 1 year
License: Required for submission
Certifications: Basic Life Support (BLS) required
Must-Have:
- Home Health & Hospice experience
- Current Driver's License & Auto Insurance
Job Description:
- Conduct initial assessments of clients and families to determine home health needs, including complete physical assessments and histories of current and previous illnesses.
- Regularly re-evaluate client nursing needs and use health assessment data to determine nursing diagnoses.
- Initiate and revise care plans as client status and needs change.
- Develop care plans, establish goals based on diagnosis, and incorporate therapeutic, preventive, and rehabilitative nursing actions.
- Include clients and families in the planning process.
- Administer medications and treatments as prescribed by physicians.
- Provide health care instructions to clients as appropriate per assessment and plan of care.
- Identify discharge planning needs as part of care plan development and implement them before client discharge.
- Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate client care for assigned caseload.
- Prepare clinical notes and update the primary physician at least every sixty days or as necessary.
- Communicate with physicians regarding client needs, report any changes in condition, and obtain/receive physician orders as required.
- Ensure arrangements for equipment and other necessary items and services are available.
- Instruct, supervise, and evaluate home health aide care provided.
- Serve clients within a set radius, providing care to patients in homes, nursing homes, etc.
Additional Information:
- The position requires coverage in a specified radius area.
- Candidates should have a reliable mode of transportation.
- On-call rotation includes evenings, weekends, and holiday coverage as required.
