Job Description
Job Title: Physical Therapist - Home Health
Job Specialty: Therapy/Rehabilitation
Job Duration: 13 weeks
Shift: Day Shift 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: No
Experience: Minimum 1 full year of Adult Home Health Experience
License: Must have license in hand unless the license takes a few days to obtain
Certifications: CPR, BLS
Must-Have:
- Reliable transportation
- Current driver's license
- Agency-required liability insurance
Job Description:
- Conduct home visits to patients and attend periodic office meetings
- Average of 5-6 patients per day, assessed based on point values from different activities such as visits, documentation, and travel
- Handle cases including Start of Care and Follow-Ups
- Must be willing to travel up to 75 miles from the home clinic, with travel between patient homes reimbursed at the IRS standard rate
- Use EMR systems Pointcare/Home Care Home Base for documentation
- Responsible for completing all documentation within 24 hours of the patient appointment
- Assigned to a territory covering parts of Northeast Indiana, including Allen, Whitley, Wabash, Huntington, Wells, Adams, Grant, Blackford, and Jay counties based on patient need
- In the event of low patient census, the clinician may float to a nearby clinic to meet financial needs
- Engage in occasional weekend work as required by the location and profession
- Be aware that there is no on-call requirement for this position
- Ensure a minimum of 30 productivity points per week, with 1 Start of Care equaling 2.5 points
