CDPC is seeking three Clinical Nurse Assessors to work in the Crisis Unit for the Night and Evening shifts. Contract will be award based on best value including interviews and past performance.
Minimum Qualifications: Candidate must possess a license and current registration as a registered professional Nurse in New York State.
Agency Clinical Nurse Assessor Responsibilities: Develop psychosocial assessments, provides crisis intervention, records significant psychosocial events and/or changes in clinical condition, and determine the individual's strengths and need for linkage and referral services.
AGENCY CLINICAL NURSE ASSESSOR DUTIES and GERERAL RESPONSIBILITES
Responding to emerging and emergency medical/physical and psychiatric conditions.
Participate in staff training, team communications, maintenance of medical records, and conveyance of verbal and written hand off communication. Maintains clear, concise, and accurate reports.
Proficient and skilled in best practices within the scope of the professional registered nurse, including, but not limited to infection control, medication administration, education, and medication management. Keeps accurate records of all narcotics within their shift.
Provides administrative and clinical management and accepts the role of shift charge as indicated.
Maintains current with training (mandated and best practice), attends discipline meetings, reads minutes, and familiarizes in CDPC policies and guidelines. Assess individual and family needs, strengths, and limitations.
Develop psychosocial assessments.
Provides crisis intervention.
Record significant psychosocial events and/or changes in clinical condition.
Determine the individual's strengths and need for linkage and referral services.
Serve as a resource to other team members regarding social adjustment, discharge service needs, and the effective utilization of community services.
Determine discharge service needs and appropriate community resources.
Educate individual and family about the nature of the disorder/illness and available resources and how to access them.
Conduct clinical assessments to determine appropriate therapeutic interventions, services and supports, including diagnostic impressions.
Prepare reports and maintain medical and other case records including progress notes, observations, recommendations, and discharge plans.
Attend treatment team meetings and actively present clinical information and observations to staff in other disciplines.
Serve as a primary point of contact for information and coordination of services and treatment among several groups including: the individual and family members; vocational and educational service providers, other social services entities; probation, parole, and other criminal justice entities; and other community agencies associated with treatment.
Collaborate with local hospitals for patient transfer for continued hospitalization
and treatment.
