Clinica Sierra Vista, accredited by the rigorous Joint Commission, serves patients at 31 health centers in Fresno and Kern counties. Our services include Pediatric Care, Adult Care, Dental Care, Behavioral Health, Case Management, Prenatal & Women's Health, Substance Abuse Treatment, HIV/AIDS testing and treatment, Health Education, Outreach and WIC services. We're looking for someone to join our team as a Physician Geriatrician who:
Provides general outpatient geriatric services. The physician must demonstrate experience working in a diverse environment and be flexible, knowledgeable, and compassionate towards the care of geriatric patients. Will provide service to the geriatric population of the organization and those referred by our primary care providers in the 20+ primary care centers. This is an excellent opportunity to work within a warm, caring atmosphere where the health of our patients is of top priority.

We're looking for someone to join our team as a Physician Geriatrician who:

Essential Functions:
- Performs in person and virtual visits.
- Conducts comprehensive Medicare Annual Wellness visits.
- Collaborates with other healthcare professionals, such as primary care providers, psychiatrists, psychologists, and social workers, to provide comprehensive care to patients.
- Keeps accurate and up-to-date patient records in compliance with legal, ethical, and organizational standards.
- Always adheres to the highest standards of medical ethics.
- Always assures quality of care for all patients and regularly participates in clinic Peer Review and Quality Assurance Programs
- Clinic practice of at least 34 patient care hours per week, with 6 hours of admin time built into the 40 hour per week schedule.
- Opportunity to partially work in academics with the Family Medicine residency program if interested.
Other Responsibilities:
- Supervises advance practice providers (Nurse practitioners and/or Physicians Assistant's) on- site and by telephone with outlying clinic sites.
- Represents CSV in professional groups.
- Assists the Chief Medical Officer in design, implementation, and evaluation of health care programs/protocols for patients and staff.
- On a scheduled basis may be required to work at any satellite facility.
- Reasonable and predictable in person attendance.
- Other duties as required.
