- Promote continuity of care and cost-effectiveness through the integration of case management, utilization review, management, and discharge planning functions.
- Serve as a patient advocate by addressing the holistic needs of patients and managing resource utilization and discharge planning.
- Experience with electronic medical records (EMR) preferred.
- Ability to work collaboratively within a team environment.
