Job Title: Physical Therapist

Job Specialty: Home Health Physical Therapy

Job Duration: 13 weeks

Shift: 8-hour days, Monday to Friday, 8:00 am to 4:30 pm, no call

Guaranteed Hours: 40 hours per week

Experience: Minimum of 2 years

License: Required to submit

Certifications: Basic Life Support (BLS)

Must-Have:

- Experience in home health physical therapy
- Valid physical therapy license
- BLS certification

Job Description:

- Provide home health physical therapy services to patients.
- Develop and implement personalized care plans for each patient.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients and their families on exercises and therapies to aid in recovery.
- Maintain accurate and up-to-date patient records.
- Adhere to all professional guidelines and standards for physical therapy practice.
- Communicate effectively with patients and team members to achieve optimal outcomes.
