Must-Have: 3-5 years of recent acute discharge planning experience in an inpatient hospital setting

Job Description:

- Perform assessments and discharge planning for patients with diverse needs.
- Coordinate with various units, primarily focusing on oncology, med-surg, and ICU.
- Work every other weekend.
- Create and implement discharge plans for patients requiring different types of support, including:
- Nursing home placements.
- Home health services.
- Support for homelessness.
- Crisis management for uninsured patients.
- Occasionally cover shifts at a sister hospital.
- Behavioral health treatments, counseling, or interventions are not included in this role.
