Job Description:
- Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.
- Serve a diverse patient population, ranging in age from birth to over 100 years.
- Collaborate with patients, their families, physicians, and other allied health professionals to identify patient needs and coordinate services to meet those needs.
- Utilize thorough and timely electronic documentation and maintain interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.
- Participate in weekend rotations once every 6 weeks.
- Must have the ability to travel to assigned cases.
- Serve various regions and neighborhoods.
