Job Description
Job Title: Registered Nurse
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day, 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year of adult home health experience or 1 full year of hospice experience
License: Must have a valid nursing license in hand
Certifications: CPR, BLS
Must-Have:
- Proficiency in HomeCare HomeBase (HCHB) with usage within the last 6 months
- Experience with OASIS assessments
Job Description:
- Coordinate and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible to Clinical Manager for direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Travel to home visits and attend periodic office meetings; reliable transportation, a current driver's license, and agency-required liability insurance are essential.
- Average of 5-8 patients per day with a point system in place to track productivity; 30 points per week are expected, with each visit and activity holding different point values.
- Types of cases include Start of Care, Wound Care, and Follow-up Visits.
- Must be open to weekend and on-call rotations approximately 1-2 times per month.
- Will cover a territory with a mileage radius of up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate.
- Must have a vehicle and be comfortable with traveling between patient homes and conducting documentation within 24 hours of each appointment.
- Documentation must be completed on time to avoid impact on billing and follow-up patient care.
- The option to float to nearby clinics to pick up hours if patient census is low.
