Description:

The Senior Director of Population Health holds a key leadership role in the Population Health department, supporting the VP in developing and executing the organization's strategy to improve population health outcomes. This role is responsible for operational leadership, program implementation, and ensuring alignment between strategic initiatives and day-to-day execution. The Senior Director collaborates with cross-functional teams, payer organizations, and community stakeholders to advance value-based care models, health equity efforts, and data-driven decision-making.
  • Strategic Execution: Translate high-level population health strategies into actionable programs and initiatives.
  • Operational Leadership: Oversee the implementation of care management, data analytics, and risk stratification programs to enhance population health outcomes.
  • Performance Monitoring: Develop and track key performance indicators (KPIs) to evaluate program effectiveness and drive continuous improvement.
  • Stakeholder Collaboration: Partner with internal departments, healthcare providers, payers, and community organizations to enhance care coordination and patient-centered approaches.
  • Regulatory Compliance: Ensure programs adhere to regulatory requirements, accreditation standards, and best practices in population health management.
  • Financial Oversight: Manage budgets and resource allocation for population health initiatives to ensure cost-effectiveness and sustainability.
  • Leadership & Team Development: Mentor and lead a team of population health professionals, fostering a culture of innovation and excellence.
  • Innovation & Best Practices: Stay informed of industry trends, emerging technologies, and innovative care models to drive continuous enhancement of population health efforts.
  • Manage and supervise assigned employees / teams, including work assignments, attendance, discipline, performance evaluations and planning requirements.
  • Perform other related duties at the discretion of the Vice President.


  • Education: Master's degree in healthcare administration, public health, business administration, nursing, or a related field required.
  • License(s): Not required
  • Experience/Skills Required:
    • Minimum of 5 years of progressive leadership experience in population health, care management, or healthcare operations.
    • Strong expertise in value-based care models, risk stratification, and healthcare analytics.
    • Proven experience in managing large-scale programs and leading cross-functional teams.
    • Excellent communication and relationship-building skills with the ability to influence internal and external stakeholders.
    • Knowledge of healthcare regulations, quality metrics, and social determinants of health frameworks.
