Job Description
Job Title: Registered Nurse (RN)

Shift: 8-hour Day Shift, 8:30 AM - 5:00 PM, Monday to Friday
Guaranteed Hours: 40 hours per week
Experience: Minimum of 1 year of hospital case management with discharge planning experience required
License: Active DC RN license
Certifications: Bachelor of Science in Nursing (BSN)
Must-Have: Knowledge of Cerner

Job Description:
- Collaborate with a multidisciplinary team to develop, refine, update, and communicate the plan of care for a defined patient population.
- Implement clinical pathways that include educational needs of patients and families.
- Monitor and update the plan of care for any deviations.

Please note, candidates must live at least 100 miles away from the job location.
