License: Registered with the Board of Registration in Massachusetts

- Proficient computer skills with the ability to learn new software applications
- Experience working in an interdisciplinary team
- Strong verbal, written, and electronic communication skills
- Excellent interpersonal and customer service skills with the ability to work with a diverse population
Job Description:
- Provide speech therapy services to patients in their homes who have impairments, functional limitations, or disabilities due to injury, disease, or other causes.
- Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services to meet those needs.
- Manage technical, environmental, and financial resources efficiently.
- Complete age-appropriate evaluations that reflect patient history, relevant systems review, and appropriate tests and measures; report findings to the primary clinician.
- Participate in weekend rotations every six weeks.
- Reassess and modify the treatment plan at least every two weeks to align with discharge goals; communicate changes to patients, primary caregivers, and the interdisciplinary team.
- Ensure physician orders are current and consistent with the plan of care and documented in the medical record.
- Submit patient schedules to the rehab scheduler by the end of the day Thursday.
- Orient and supervise Home Health aides following agency and state guidelines.
- Manage patient caseloads in accordance with agency, state, and professional guidelines.
- Complete and export documentation timely according to agency standards.
- Update POC devices weekly and as directed.
- Verify Payor information on admission/evaluation within agency and regulatory guidelines.
- Maintain and transmit documentation to support Managed Care authorizations/DME requirements within designated timeframes.
- Consistently schedule visits according to authorizations by Managed Care payors.
- Discuss utilization/caseload with the Rehab Manager on an ongoing basis and make changes toward stated goals.
- Meet reportable outcomes and agency goals.
- Identify realistic, objective, time-oriented goals for patient performance based on examination findings.
- Select treatment and discharge plans based on examination findings and patient goals.
- Inform patients of their weekly schedule and any changes necessary.
