Job Title: Hospice Registered Nurse (RN)
Job Specialty: Hospice Care
Job Duration: 13 weeks
Shift: M-F 8a-5p, 5x8 Hr
Guaranteed Hours: 40 per week
Experience: Minimum of one year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered as experience in individual cases.
License: Current license to practice nursing in the specified state
Certifications: BLS (American Heart Association), current automobile insurance, and valid driver's license
Must-Have:
- Knowledge of terminally ill patients and hospice care
- Ability to apply knowledge of the special needs of hospice patients and families
- Excellent patient assessment, communication, and documentation skills
- Familiarity with community resources, managed care principles, and regulatory guidelines
Job Description:
- Provide direct patient care services to hospice patients and supportive care to their families
- Manage an interdisciplinary group, including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff
- Assure the highest quality of care is provided and documented in accordance with the Plan of Care
- Perform initial and ongoing assessments of the patient
- Develop and implement the Hospice Plan of Care in collaboration with the physician and other Interdisciplinary Group members
- Work closely with Interdisciplinary Group members to offer comprehensive services including physical, psychosocial, educational, emotional, and spiritual support
- Monitor scheduling and participate in the Quality Assessment and Performance Improvement Program
- Educate patients, families, or other caregivers in relation to needs identified on the Plan of Care
- Supervise Aides and perform home visits at least every two weeks
- Prepare written assignments for Aides and maintain accurate and updated medical records
