Job Title: Therapist MSW
Job Specialty: Mental Health and Therapy
Job Duration: 13 weeks
Shift: Full-Time; Monday to Friday 8:30 am-5:00 pm
Guaranteed Hours: 40 per week
Experience: Minimum of two years' experience in clinical assessments and crisis intervention, preferably in an acute healthcare setting. Must have at least one year of hospice experience. Experience with culturally diverse patients required.
License: Master's Degree in Social Work (MSW) or Marriage and Family Therapy (MFT) from an accredited school
Certifications: BLS required (AHA)
Must-Have: Hospice experience, experience in acute care settings
Job Description:
- Provide comprehensive assessments, coordination, planning, and implementation of psychosocial treatment plans for patients in various departments such as Community Health, Palliative Care, Cancer Center Community Program, Home Health, and Hospice.
- Promote optimal mental health by offering ongoing psychosocial support and services to patients and families for adjustments to illness, grief, addiction, and other psychosocial stressors.
- Collaborate with an interdisciplinary team to achieve treatment goals.
- Utilize various factors to evaluate patients' biopsychosocial needs and develop comprehensive treatment and discharge plans.
- Document objective and accurate data on assessments and interventions.
- Maintain a working knowledge of community-based resources, programs, and social services.
- Educate patients and families about healthcare options and connect them with appropriate resources.
- Develop and facilitate support groups and workshops as needed.
- Serve as a resource and preceptor to MSW and MFT interns.
- Provide input on treatment schedules, patient assignments, and crisis interventions as requested by the Clinical Manager.
- Perform other duties as assigned.
- Travel to patient homes is required, with 4-6 visits per day preferred.
Preferred Skills & Experience:
- Experience in clinical assessments and crisis intervention in a hospital or acute care setting.
- Bilingual or multilingual ability is preferred.
