Responsibilities
- Appointment & Care Coordination: Manage appointment schedules and ensure seamless transitions between medical transport and care, adapting quickly to changing circumstances.
- Direct Medical Support: Administer medications, check vital signs, and provide necessary medical care during transport, adjusting as needed.
- Safety & Supervision: Ensure the safety of residents during transport and appointments, managing time effectively across multiple residents.
- Communication & Advocacy: Maintain clear communication with healthcare teams, families, and transportation staff, keeping everyone informed and involved in the care process.
- Documentation & Administrative Duties: Accurately document all care provided, update care plans, and assist with quality improvement initiatives.
Qualifications
- LPN or RN with 3-5 years of relevant experience
- CPR/First Aid Certification required
- Valid driver's license (Class C or D)
- Bachelor's degree preferred
