Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Guaranteed Hours: 40
Experience: 3+ years of current experience within the past six months required
License: Current New Hampshire license
Certifications: Basic Life Support (BLS)
Must-Have:
- Own form of transportation
- IV certification required
- COVID vaccination series and booster
Job Description:
- Observe each resident continuously, noting changes in mental and physical well-being. Notify the supervisor of observations and discuss the need to inform the physician.
- Receive and provide detailed resident status reports during shift changes.
- Accurately receive, transcribe, and carry out physician's orders.
- Administer medications and treatments as ordered by the physician and document using a computerized system.
- Order and maintain all pharmaceuticals for the units.
- Supervise LNAs, ensuring compliance with licensing requirements.
- Assign and measure nursing staff resident care according to abilities and scope of practice, supervising care and assisting direct-care staff as needed.
- Accompany physicians on resident rounds, addressing medical needs and facilitating communication.
- Administer oxygen and gastrostomy/jejunostomy feedings per facility guidelines.
- Perform wound care and manage wound vacs.
- Perform regular unit duties, including checking appointments, activity boards, and staff reminders for ambulation lists and nutritional documentation.
- Monitor LNA documentation, ensure infection control practices, and maintain a clean, hazard-free unit.
- Report any issues through the chain of command to the Nurse Manager or Nursing Supervisor.
- Admit, discharge, and transfer residents, completing necessary documentation and notifications.
- Complete 24-hour reports, check new orders, and maintain communication regarding resident issues.
- Communicate with residents' families to resolve concerns and obtain consent when necessary.
- Manage incoming calls and determine appropriate responses.
- Assume responsibility for the management of assigned units during shifts under the direction of the Nursing Supervisor or Director of Nursing Services.
- Ensure compliance with state and federal regulations for controlled medications.
- Assist with the orientation and continuing education of new nursing employees and staff.
- Encourage and assign staff to attend educational in-service meetings.
- Evaluate nursing staff according to facility policies and procedures and recommend disciplinary actions if necessary.
Note: Candidates must live 50 miles away from the facility and be willing to work during the Christmas-New Year block if needed. Exemptions for COVID vaccination are not accepted, and proof must be uploaded. Requested time off must be noted and verified during upload, with no more than five days accepted.
