Job Title: LVN / LPN
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day, 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year Adult Home Health or Hospice experience
License: Must have license in hand
Certifications: CPR, BLS
Must-Have:
- Proficiency in HomeCare HomeBase and Oasis EMR within the last 6 months
- Reliable transportation with current driver's license and liability insurance
Job Description:
- Provide, coordinate, and direct the provision of home nursing care based on physician's orders and agency policies and procedures.
- Responsible to the Clinical Manager for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and participate in periodic office meetings.
- Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS Standard rate.
- Handle various types of cases including Start of Care, Wound Care, and Follow-up Visits.
- Documentation must be completed within 24 hours of patient appointments.
- Open to on-call rotations as required.
- Clinician must accept all scheduled patients, regardless of distance, and maintain weekly productivity points.
- Opportunity to float to nearby clinics to pick up additional hours if patient census is low.
