Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None
Experience: Minimum 1 full year of Adult Home Health or Hospice experience
License: Must have license in hand
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase (HCHB) used within the last 6 months, a valid driver's license, reliable transportation, and current auto insurance
Job Description:
- Provide, coordinate, and direct home nursing care per physician orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible for supervising and evaluating Home Health Aides and other Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Handle cases including Start of Care, Wound Care, and Follow-up Visits.
- Productivity expectation is to achieve 30-35 points per week. Each type of visit and activity such as documentation and travel holds a different point value. The contractor will be assigned between 5-10 patients per day to equate to roughly 6-8 points.
- The mileage for home visits is reimbursed at the IRS standard rate. Travel is required up to a 75-mile radius from the home clinic.
- Local candidates are accepted at the same rate.
- Proficient charting using the HomeCare HomeBase (HCHB) tablet system; RNs, PTs, and OTs are required to be proficient in Oasis.
- Complete all patient documentation within 24 hours of the appointment.
- Participate in floating to nearby clinics if necessary to meet weekly productivity.
- There is no on-call requirement for LPNs.
