The Section of Geriatrics and Palliative Medicine at Baylor College of Medicine is dedicated to excellence in patient care, education, scholarly activities, and community engagement. As geriatricians we apply scientific knowledge, clinical expertise and compassion to prevention, screening, diagnosis, and treatment of adult elderly patients with a broad spectrum of medical conditions. We care for diverse patients in senior care outpatient settings. Our collaborative interdisciplinary teams practice at Baylor Medicine Clinic, Baylor St. Luke's CHI Hospitals, Houston Methodist Hospital, Harris Health System (Ben Taub Hospital, Vallbona Health Center, and the House Call Service) and Michael E. DeBakey Veterans Affairs Medical Center. Our section also provides care in several facilities.
The educational mission of the Geriatrics Section is critical. The Geriatrics fellowship is one of the largest in the nation, and our palliative care fellowship is affiliated with MD Anderson. We also teach medical students, internal medicine and family medicine residents, physician assistant students and PA residents.
Summary
The Section of Geriatrics and Palliative Medicine at Baylor College of Medicine is seeking a full-time Clinician/Educator for the House Call Service. We serve as primary care physicians for the vulnerable, homebound elderly population of Harris County and perform one-time visits for home safety evaluations and transition of care visits. The ideal candidates are patient-centered and committed to excellence in providing quality patient care and teaching.

Job Duties
- Provide medical care to elderly adults in the home setting.
- Coordinate with other disciplinary care teams for safe discharge of patients.
- Participate in any team meetings. Work with the team to develop the appropriate order sets and procedures that are needed.
- Take telephone calls several weeks per year.
- Contribute to quality improvement and research projects. Collaborate nationally with known leaders in the field. Maintain continuing medical education as per college and section requirements.
- Renewing and meeting all required licenses and certification requirements.
- Document within EPIC- electronic medical records and document all patient encounters as needed.
- Complete all documentation in a timely manner so that billing and other administrative tasks can happen smoothly to ensure a robust operational system.
- Bilingual (Spanish) speakers will be given preference.

Minimum Qualifications
- Education Requirement: MD, DO; Geriatrics Board Certified.
- Licensure Requirement: Licensed by the Texas Medical Board.
