Job Title: Licensed Practical Nurse (LPN) - Full Time
Job Specialty: Long-Term Care (LTC)
Job Duration: 13 weeks
Shift: Full-Time Day/Evening rotation (5x8 per week, every other weekend, holidays)

Experience: 1 year as an LPN in an LTC Facility
License: New York LPN Licensure
Certifications: BLS (Certification through AHA or RQI)
Must-Have:
- 1 year of experience as an LPN
- Willingness to work every other weekend and on holidays
Job Description:
- Provide direct and indirect skilled nursing care to assigned residents under the supervision of a Registered Nurse.
- Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
- Contribute to developing an age-appropriate and culturally relevant care plan that reflects nursing diagnoses and interventions to achieve desired resident outcomes.
- Recommend modifications to the care plan based on resident outcomes and make referrals as needed to ensure comprehensive resident care.
- Maintain collaborative relationships with residents, families, and healthcare team members to ensure customer satisfaction.
- Stay updated with current knowledge and competency in Nursing Practice through continuing education and in-service programs.
- Participate in mentoring and orientation of new staff and consider safety, cost, and impact factors in care planning and delivery.
- Provide leadership in the professional practice setting under the direction of the RN.
- Anticipate potential occupational exposure to blood-borne pathogens through tasks involving contact with blood, body fluids, or tissues.
