Job Description
Job Title: Social Services Case Manager
Job Specialty: Masters of Social Work (MSW) - Case Management
Job Duration: 13 Weeks
Shift: Day, 5x8-Hour (08:00 - 16:30)
Guaranteed Hours: Not Specified
Experience: Prior case management and discharge experience required. Utilization management experience is helpful but not required. Travel experience and an average daily census of 18-20.
License: Masters in Social Work and Washington-issued post-MSW license such as the licensed social work associate independent clinical.
Certifications: BCLS required
Must-Have:
- Acute hospital experience
- Experience with patients with communication problems (hearing/speech)
- Knowledge of community programs (income/insurance) and community-based service programs
Job Description:
- Must have either an associate clinical license or LCISW
- Requires experience with admission criteria, initial evaluation, developing plans of care, care coordination, and discharge planning
- Utilize InterQual and Milliman Guidelines, appeals, denials, concurrent and continued stay reviews
- Responsible for assessing response to treatment and conducting biopsychosocial assessments
- Coordinate services for home care, facilitate care conferences, and make referrals to rehabilitation and long-term care facilities
- Required documentation according to the plan of care, including the delivery plan and goals
- Experience with documentation reflecting understanding of coverage criteria
- Familiarity with major disease processes such as dementia and stroke, with preferred experience in cancer, CHF, COPD, diabetes, HIV/AIDS, and psychiatric disorders
- Knowledge of CMS regulations, CPT coding and billing, ICD 10 Coding, and other healthcare standards
- Proficiency in charting software like Epic and Cerner
- Willingness to float and take on care manager responsibilities including care coordination and discharge planning
