Job Description
Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
Job Specialty: Long Term Care
Job Duration: 13 weeks
Shift: 8-hour day shifts
Guaranteed Hours: 40 hours per week
Experience: 3+ years of recent experience required (within the past 6 months)
License: Current New Hampshire Nursing License
Certifications: Basic Life Support (BLS) certification
Must-Have: COVID Vaccination Series and Booster required (Exemptions not accepted)
Job Description:
- Observe each resident continuously, noting changes in mental and physical well-being, and report to the Supervisor as necessary.
- Receive Resident Status Reports from previous shifts and provide detailed updates to incoming shifts.
- Accurately receive, transcribe, and carry out physician's orders using a computerized system for medication administration records (MARs)/treatment administration records (TARs) and maintain electronic documentation.
- Administer medications and treatments as ordered by the physician; order and maintain pharmaceuticals for the units.
- Supervise Licensed Nursing Assistants (LNAs) following licensing requirements from the New Hampshire Board of Nursing and assist direct-care staff with nursing care as needed.
- Conduct rounds with physicians to address residents' medical needs and facilitate communication during 60-day reviews.
- Administer oxygen, gastrostomy/jejunostomy feedings, and perform wound care as per facility guidelines.
- Perform regular duties including checking appointments, monitoring therapy appointments, updating documentation, and ensuring infection control practices.
- Manage unit cleanliness and safety, ensuring it is hazard-free.
- Follow the chain of command, reporting any issues to the Nurse Manager/Nursing Supervisor.
- Encourage and involve LNAs in observations, documentation, and reporting.
- Handle admissions, discharges, and transfers of residents, completing necessary documentation and notifications.
- Complete 24-hour reports for assigned units; maintain consistent communication with nursing leadership regarding resident issues.
- Communicate with residents' families to resolve concerns, notify of new orders, and obtain necessary consents.
- Manage incoming calls and determine appropriate responses.
- Assume responsibility for managing assigned units during shifts under the direction of Nursing Supervisors or Directors.
- Comply with State and Federal Regulations for controlled medication management, including end-of-shift counts and discrepancy reporting.
- Assist with the orientation of new nursing staff and encourage continuing education and in-service attendance.
- Evaluate staff performance according to facility policy and recommend disciplinary actions as necessary.
- Required to work every other weekend and holiday coverage, with willingness for potential holiday block work if needed.
