Responsibilities

Dispensing Job Functions
Able to analyze, interpret and organize complex information and determine the appropriate courses ofaction. Consistently follows applicable practice standards, policies and procedures, rules and regulations andother applicable standards. Maintains concise, accurate and complete patient's electronic medication profiles. Demonstrates sound professional judgment consistent with clinical/academic background. Interprets physician's orders accurately. Consults with practitioners and professional staff as needed to clarify or to ensure positive therapeuticoutcomes and to minimize drug-related problems/errors. Labels and dispenses drugs accurately and completely, and in a timely manner. Reviews patient profiles for any interactions based on diagnosis, allergies or duplications via orderentry and appropriately responds to electronic alerts. Demonstrates appropriate sterile aseptic technique, order of mixing, use of compounding equipmentand other pharmaceutical preparation practices. Ensures dosing is appropriate for patient's weight and age. Maintains, monitors and reports adverse drug reactions. Completes medication orders for delivery to nursing units in a timely fashion. Utilizes therapeutic substitution protocols and promotes rational, cost- effective therapeuticalternatives. Effectively organizes and prioritizes work and ensures timely and effective department operation. Demonstrates the ability to make critical decisions in a timely manner. Adapts positively to frequent interruptions and changes in workload and/or work schedule. Coordinates and reviews technician activity from a management perspective, as needed. Demonstrates a basic knowledge of the automated dispensing machine (ADM) technology. Ensures that the operation of the automated dispensing system, computerized patient profiles,controlled substance software, and other software and hardware is working effectively, safely and accurately. Assists in maintaining proper inventory levels. Ensures that narcotics/controlled substances and corresponding records are handled appropriately. Verifies accuracy of all products prepared for patients by technicians. Maintains positive professional communications and attitudes with all hospital staff. Assists the DOP with special projects as requested.Answers phone in courteous and professional manner. Attends pharmacy staff meetings and in-services. Reconciles and restocks emergency cart drugs and other floorstock-type systems in a timely fashion. Assists in teaching of all new pharmacy employees. Reports unusual occurrences to the Clinical Coordinator and completes appropriate paperwork. Maintains QI program, as required.

Clinical Job Functions
Able to analyze, interpret and organize complex information and determine the appropriate courses ofaction. Demonstrates sound professional judgment consistent with clinical/academic background. Provides clinical interventions on a regular and accurate basis. Monitors medication therapies identified in the pharmacy department policy. Performs pharmacokinetic monitoring according to protocol, including appropriate ordering andinterpretation of labs and ordering of drug regimens. Consults with practitioners and professional staff as needed to clarify or to ensure positive therapeuticoutcomes and minimize drug-related problems. Recommends appropriate changes in therapy. Checks all neonate and pediatric doses ordered. Recommends appropriate drugs to use and avoid for the neonate, pediatric, child-bearing age female,and geriatric patient. Appropriately and accurately assesses patient clinical information. Reads, extracts and interprets information in the patient chart accurately and interprets subjective andobjective data correctly. Supports the formulary of recommended drugs and minimizes the procurement of non-formularyproducts. Provides drug information as requested. Able to quickly summarize complex information from avariety of sources. Provides constructive ideas and comments regarding ways to improve quality and medication use.Willing to implement ideas when DOP approval is given.

Documentation
Collects data for quality improvement or assessment when needed. Maintains concise, accurate and complete pharmacokinetic and CrCl profiles when applicable. Maintains concise, accurate and complete documentation in the patient's chart. Maintains all documentation logs and records completely. Documents medication errors. Documents adverse drug reactions. Documents clinical interventions for improved patient care when requested.

Performs Other Duties as Required
Attends mandatory in-services, staff meetings, and actively participates in self-education. Completes all required competency assessments on time. Reads and initials all posted required departmental and hospital correspondence. Maintains required job knowledge (e.g. licensure and credentials, continuing education etc.). Performs other duties as required.

Knowledge, Skills & Abilities

Light Work: Frequent lifting and/or carrying of objects weighing up to10 lbs with a maximum lifting of 25 lbs.

Training in proficiency of age-specific therapies for ages served by the pharmacy,which include neo-natals, infants, juveniles, adolescents, adults and geriatrics.
