Position Summary
The Clinical Care Manager delivers indirect patient care in a healthcare office, demonstrating professionalism, initiative, and responsibility with minimal supervision. They prioritize care based on physical, psychosocial, and system flow factors, while communicating with the primary care team about patient status, symptoms, and diagnostic results. The role requires the ability to swiftly and accurately respond to changes in patient conditions or treatment outcomes.

The work environment is characteristic of an office environment. While performing the duties of this job, the employee is frequently required to sit; use hands to handle or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

Required Qualifications (Knowledge, Skills, Abilities, Education, Training, and Licensure)
- Graduate of an accredited school of Nursing with up to six (6) months Registered Nurse experience.
- Current Registered Nurse (RN) license within the state of California.
- Previous nursing experience in Case Management.
- Demonstrate clear and concise communication skills both verbally and in writing and demonstrate a high level of listening skills.
- Computer (PC) literate in general office functions such as MS Word, access, email and preparation of spreadsheets.
- Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
- Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
- Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
- Fluency in additional languages preferred.
- Enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops; show responsibility for own professional practice and ongoing education.
- Support and maintain a culture of safety and quality.
Essential Responsibilities and Duties
- Provide indirect patient care, evaluate outcomes, consult with other specialists as required and adjust nursing care processes as indicated to ensure optimal patient care.
- Attend annual review and department in services, as scheduled; staff meetings as required.
- Actively participate in performance improvement and continuous quality improvement (QA/QI) activities.
- Maintain accurate and continued nursing documentation, including patient history, conditions, treatments, responses, and assessment of changes.
- Serve as a liaison between provider and pharmacy for questions on dosages/alternative meds, etc, answer patient questions regarding medication.
- Respond to and complete requests from Home Health Agencies per an established protocol and obtain provider signatures when required.
- Review patient forms with the provider, complete them (when appropriate), and obtain the provider's signature per established protocol.
- Identify and address psychosocial and cultural needs of patients and family-functions as a liaison between administration, patients, physicians, and other healthcare providers.
- Demonstrate knowledge of growth and development principles over the life span and the skills necessary to provide appropriate age care to the patients served. Able to interpret data about the patient's status to identify each patient's age-specific needs and provide the care needed by the patient group.
- Interact professionally with patients and family and involve patients and family in forming the plan of care.
- Maintain current knowledge of medications and correct administration based on the age of the patient clinical condition.
- Plan, develop, assesses, and evaluates care provided to members. Develop and maintain case management policies and procedures.
- Coordinate the interdisciplinary approach to providing continuity of care.
- Arrange and monitor follow-up appointments. Encourage members to follow prescribed course of care (e.g., drug therapy, physical therapy). Make referrals to appropriate community services
- Identify and recommend opportunities for cost savings and improving the quality of care across the continuum
- Act as liaison for outside agencies, non-plan facilities, and outside providers. In conjunction with physicians, evaluate and develop treatment plans, recommend alternative levels of care, and ensures compliance with federal, state, and local requirement.
