Job Title:
Care Coordinator (Case Manager) Nurse
Job Specialty:
Care Manager, Manager-Case Management, Clinic
Job Duration:
13 weeks
Shift:
Monday through Friday, 8:00 AM to 5:00 PM, with a one-hour lunch break. No weekends or holidays.
Guaranteed Hours:
36 hours per week
Experience:
Minimum of 3 years in Case Management
License:
Registered Nurse license
Certifications:
N/A
Must-Have:
Three years of experience in Case Management
Job Description:
- Work within the Primary Care clinic as a Care Coordinator.
- Collaborate with support nursing staff and providers for efficient patient care.
- Orientation sessions occur on Mondays; missing the compliance deadline will result in a rescheduled orientation to the next available Monday.
- Occasional requirements to float to other departments based on qualifications and experience, which may include taking a patient load or performing task-oriented activities such as answering call lights and phones.
- Travelers may be asked to take on charge duties with no additional charge pay.
- Ensure completion of all onboarding and compliance requirements promptly.
